2 Eliquis and Vyndaqel: Medicare Price Setting and the 2028 Generic Date

  • Eliquis $2.52bn in Q3 2026, +25.2%: US $1.81bn, +39.9%; international $715m, (1.1%), with generics and price erosion in some markets. 9M 2026 $7.11bn, +19.7%. Pfizer attributes the growth mainly to higher US net price from lower rebates and channel mix.
  • Medicare price and patents: the Medicare negotiated price for Eliquis took effect on 1 Jan 2026 and also applies to 340B dispensing to Medicare patients; the composition patent expires Nov 2026, settled generic makers may launch on 1 Apr 2028 and the formulation patent runs to 2031. The Azurity formulation case settled in Oct 2026; a suit against TAHO (oral film) followed.
  • Vyndaqel family $1.77bn in Q3 2026, +11.4%: US $1.09bn, +14.6%, international $687m. 9M 2026 $5.14bn, +9.5%.
  • Vyndaqel exclusivity: US basic patent 2026 with an interim extension to Dec 2026 and a pending extension to 2028; Vyndamax settlements with Dexcel, Hikma and Cipla (Apr 2026) set an effective US expiry of 1 Jun 2031, and Pfizer expects US revenue to stay relatively stable from 2028 to mid 2031; litigation with Apotex continues.
  • Weight in the group: the two products earned $14.34bn in FY2025, 22.9% of revenue, and 25.9% of Q3 2026 revenue.

How the anticoagulant and ATTR markets are changing

  • Medicare price setting: CMS named Eliquis among the first 10 drugs for Medicare price setting in Aug 2023, with the price from 1 Jan 2026; the Part D redesign caps Medicare out of pocket costs at $2,100 in 2026 and requires manufacturer discounts of 10% in the initial phase and 20% in the catastrophic phase. An HHS 340B pilot from Jan 2027 is expected to include Eliquis.
  • BMS view: BMS, which books most Eliquis sales, reported worldwide Eliquis revenue of $4.69bn in Q3 2026, +25.2%, with US +39% and international (7%), and guides 2026 growth of 20% to 25%.
  • ATTR cardiomyopathy now has three marketed mechanisms: Pfizer's stabilizer Vyndaqel, Alnylam's RNAi silencer Amvuttra ($1.13bn in Q3 2026, +64.9%; 2026 TTR guidance $4.2bn to $4.5bn) and BridgeBio's stabilizer Attruby (US $253.2m, +134.2%).

How the vaccine and COVID markets are changing

  • US recommendations: ACIP moved COVID vaccination for people 6 months and older to shared clinical decision making in Sep 2025; in Aug 2026 FDA approved the 2026 to 2027 Comirnaty formula only for adults 65 and older and people aged 5 to 64 with at least one high risk condition; in Jan 2026 the CDC reduced the immunizations routinely recommended for all US children.

6 Financial Analysis & Ratios

  • Operating income in this report is computed, as Pfizer's income statement runs from costs and expenses to pretax income: revenue less cost of sales, SI&A, R&D, acquired IPR&D, amortization of intangibles and restructuring and acquisition costs. On that basis TTM Q3 2026 operating income was $13.64bn, 21.4% of revenue, against 22.8% in FY2025; before amortization and restructuring the TTM margin was 31.9%.
  • Net income TTM $4.03bn, diluted EPS $0.70, after intangible impairments of $4.4bn (Q4 2025) and $4.3bn (9M 2026), legal charges of $1.05bn and contingent consideration increases of $709m in 9M 2026, partly offset by the $1.87bn ViiV gain. TTM adjusted EPS $3.05 (company measure).
  • Cash flow TTM Q3 2026: operating cash flow $14.17bn, capex $2.39bn, free cash flow $11.79bn (18.5% of revenue), dividends $9.79bn, no buybacks; Q3 2026 operating cash flow $5.38bn.
  • Balance sheet at 27 Sep 2026: debt $62.67bn, cash and short term investments $14.27bn, net debt $48.40bn; goodwill $71.46bn and intangibles $45.96bn against $53.73bn at end 2025; Pfizer equity $89.00bn. Ratings A2 and A, both stable; $7.0bn revolver to Oct 2031 undrawn.
  • Capital returns: quarterly dividend $0.43 ($1.72 a year), declared again in Oct 2026; no buybacks in 2026 to date, with $3.3bn of authorization left; management expects leverage to stay near current levels or rise modestly through the exclusivity losses, then fall (Q3 2026 10-Q).

11 Risk Factors

  • Item 1A of the FY2025 Form 10-K holds 24 risk factors, as in FY2024 (12,511 words against 11,301): none added or removed and 7 materially reworded, led by drug pricing (MFN, TrumpRx), AI and vaccine policy. The 2026 Forms 10-Q add no new risk factor text.
Table 11.1 Risk factors and legal matters with filed figures
RiskDisclosure and change
Exclusivity losses 2026 to 2028Products with US basic patent expiry 2026 to 2028 earned $22.7bn, 36.2% of FY2025 revenue, and $6.36bn, 38.4% of Q3 2026 revenue; Pfizer expects revenue to fall from exclusivity expiries in 2026 through 2030, with the decline accelerating over the next few years.
Eliquis generic timingComposition patent expires Nov 2026 and settled generic entry is 1 Apr 2028; a loss on the 2031 formulation patent could allow launch from 21 Nov 2026.
US drug pricingMedicare negotiated prices for Eliquis from 2026 and for Ibrance and Xtandi from 2027; most favored nation agreement and TrumpRx discounts; HHS 340B pilot from Jan 2027 expected to include Eliquis, Ibrance and Xtandi.
Vaccine policyCDC cut routine childhood immunizations in Jan 2026; FDA limited the 2026 to 2027 Comirnaty formula to adults 65 and older and high risk people aged 5 to 64; Prevnar family Q3 2026 (10.2%).
AICybersecurity factor renamed and AI mentions rise from 6 to 25: AI in R&D, uncertain AI regulation, payer use of AI for prior authorization, output errors and reliance on few AI providers.
Impairments and acquisitionsIntangible impairments $4.4bn (Q4 2025) and $4.3bn (9M 2026); intangibles $45.96bn and goodwill $71.46bn at 27 Sep 2026; Metsera and Seagen named as concentrations in obesity and oncology.
LitigationComirnaty patent suits by Moderna, Arbutus and Genevant, GSK, Bayer and Sanofi; certain legal matters charges $1.05bn in 9M 2026 and about $1.1bn in FY2025.

How risk factors changed

  • Comparison: 10-K FY2025 filed 26 Feb 2026 acc 0000078003-26-000026 against 10-K FY2024 filed 27 Feb 2025 acc 0000078003-25-000054.
  • All 24 risk factor headings carry over; none added or removed; total words rise 10.7% from 11,301 to 12,511.
  • AI is named 25 times in 3 risk factors in the FY2025 10-K against 6 times in 2 a year earlier; the cybersecurity factor grows by 283 words, mostly AI text.
  • US pricing policy drives the largest edits: MFN is named 7 times in 3 factors (none a year earlier), TrumpRx once, and the pricing factor grows by 294 words.
  • Vaccine policy is new risk text: the CDC cut to routine childhood immunizations (Jan 2026) appears in 2 factors and the narrower COVID vaccine recommendations in the COVID factor.
  • Concentration and competition factors now name Metsera and Seagen as concentrations in obesity and oncology, competitors from China, and a revenue decline from exclusivity losses in 2026 to 2030 that Pfizer expects to accelerate.
